​​Our goal is to service our clients at the highest level possible. In doing so, we touch bases with you every year. We check your doctors, medications and speak with you about your personal needs and how they may change from year to year. Please complete the Medicare Questionnaire and book your appointment as this will help us to better serve you during our appointment.

Please share with us your basic contact information, your list of doctors, any medications you currently take, and your type of needs that may apply. We will do our homework prior to our meeting to make sure your needs are met.

SCHEDULE AN APPOINTMENT
After the Questionnaire, you will be asked to select a date and time on your calendar that works best for you. When your appointment arrives we will either call you or meet you online depending upon the selection you have chosen.

SCOPE OF APPOINTMENT
This form is required for those over age 65. It provides us with your permission to discuss Medicare with you BUT does not enroll or change any Medicare  plans you may currently have. This form is now required 48 hours prior to our meeting as per CMS who regulates our industry. If you need this form we will send it to your email and you can easily complete it using the online link we provide you or filling in the GENERIC SAMPLE and uploading it to our SECURE SERVER.
